Health Requirements to start school in the Lebanon Special School District:

- Tennessee Department of Health Certificate of Immunization: Parents should contact their child's health care provided or the Health Department to obtain a current Tennessee Immunization Form. Proof of up to date immunizations for age is required prior to starting school (includes PreKindergarten and Virtual School).
- Proof of a physical done within the 12 months prior to starting school for students entering PreKindergarten, Kindergarten, or first time in Tennessee school system.
