Pre-K Registration Information 2024-2025

Pre-Kindergarten Requirements
- Children must be four years of age on/by August 15, 2024
- Children must live in the Lebanon Special School District
- The Pre-K program is voluntary
- Eligibility is based on income and at-risk factors and availability
Pre-Kindergarten Parents and Child are required to attend the Pre-Kindergarten Health Fair on Tuesday, May 7, 2024 at College Hills Church of Christ on Leeville Pike Road.
Documentation Required:
- Certified Birth Certificate
- Two (2) Proofs of Residency (See Acceptable Proof of Residency in Downloadable Forms below)
- Tennessee Department of Health Certificate of Immunization (this is a document provided by a health care physician which includes shots and current physical with the doctor's signature)
- Child Custody Documentation (if applicable)
- Income Eligibility Form (See Downloadable Forms below)
- Proof of Income (See Part D of the Income Eligibility Form)

Complete the online application by clicking on the Online Registration link belowUpload the required documentation when applying or bring the documents to registration on May 7.
To access the online registration, you will be asked to create an account and then enter in information for your child. This process should take between 20-30 minutes.You can save your information and resume working on it at a later time if needed.
